Fill out the form on this page and tell us why your nominee deserves to win — the more detail, the better! To qualify, people can nominate themselves or a loved one by submitting their name and contact information, the nominee’s name and contact information, and a short explanation of why that person deserves the Gift of Hearing.

We’ll review all entries and announce our winner shortly after. The winner will be notified the week of Christmas and fitted in January.
